To promote overall good health of our BINUSIAN community, the School will be facilitating a vaccination day for anti-influenza and vitamin C booster shots from the vendor Vaxcorp.  The details of the vaccine and shot are as follows:

Demi mendukung kesehatan yang baik di komunitas BINUS, sekolah akan mengadakan vaksinasi anti-influenza dan suntikan vitamin C yang akan dilakukan oleh vendor Vaxcorp. Berikut rincian harga vaksin dan suntikan:

Kindly fill in the form if you are interested to have your child vaccinated and submit directly to your Class Teacher not later than 21 February 2020.  Vaccination will take place at B -103 (at MS across LRC) on the 28th of February, from 12:00 noon.  Payments may either be in cash, debit or credit on the day itself to be paid directly to the vendor. Please accompany your child to the venue and for the vaccination.

Silakan isi formulir terlampir jika Bapak/Ibu tertarik anak Bapak/Ibu divaksin dan kumpulkan formulir ini langsung kepada guru kelas paling lambat tanggal 21 Februari 2020. Vaksinasi akan dilaksanakan di Ruang B-103 (Gedung MS di seberang Perpustakaan) pada hari Jumat, 28 Februari 2020, mulai pukul 12.00 siang. Pembayaran bisa dilakukan secara tunai atau dengan kartu debit/kredit langsung kepada pihak yang menyediakan vaksin pada hari tersebut. Mohon bantuannya untuk menemani anak anda pada saat dilakukan vaksinasi.
